Parents: If your child has not yet visited the School-Based Health Center at Riverside or you haven’t completed the Evergreen Intake packet, please click the link below to fill out the Intake Packet for Students:

All enrolled students and their families

Faculty and staff are also welcome

Call the SBHC main line at 937-599-1411 and choose Option 5 to reach the School-Based Clinic scheduler.  Or, call the clinic directly at  937-599-1411 ext 316
