The athletic physicals will be performed free to Riverside students that will be entering grades 7-12 for the 2025/26 school year. **FORMS ATTACHED** 1. Information and Permission Slip for the Physical Day 2. Physical Form: This is an EXAMPLE of medical history forms (3 pages) and the physical forms for the doctor to complete (2 pages). Medical history must be updated on Final Forms before printing. a. Please make sure the medical history forms are signed in the areas highlighted in yellow. **These 5 pages must be printed, signed, and returned, along with the permission slip, to Athletic Director Rod Yoder or Athletic Secretary Nicole Harshbarger by WEDNESDAY, MAY 14, 2025. *Mary Rutan will not perform the physical without these forms. Flying R Winners! These ten students are our latest Flying R winners. Staff can nominate students for any good thing that they witness a student doing. All nominated students get their name , deed, and the staff member that nominated them, announced during their lunch period. Those names go into a random grid and once we get a row filled in, they win a big prize and we start all over again. These students each won a beach bag, beach towel, sunscreen, and a bag of treats. Congratulations to Elvis Purtee, Dillan Bowman, Kelsey Burt, Julia Meade, Ashley Roach, Morgan Shockey, Chance Magoto, Damien Smith, Westin Bower, and Gabe Steineman!
